Build upload forms for users to upload csv/xml/json files to mysql and use open rest APIs to query for additonal content

I'm looking for a developer that can build a php based script that provides - a user interface (form) to select a file (csv/xml/json) - use the content of that file to fill a database - use the concept... (Budget: $250 - $750 USD, Jobs: Bootstrap, MySQL, PHP)
